Private Coaching
Terms of Service

I agree to pay The ADD Management Group, LLC for Private Coaching Services as specified below.

Plan Includes:

• Three 50-minute sessions per month
• Unlimited email coaching
• Weekly check-in emails

Payment:

• $550 per month
• Payments will be automatically billed to your credit or debit card on the first of the month
• Services may be cancelled at any time
• When signing up mid-month, your first month's fee will be prorated

Missed Appointments & Cancellations:

• Appointments that are missed (“no-shows”) will not be rescheduled
• Appointments cancelled without 24 hours notice will not be rescheduled
• Unused sessions do not carry over and will not be refunded
• If you wish to discontinue coaching, your request must be provided in writing (email is acceptable) prior to charges being billed

Waiting List:

There is currently a waiting list for this coaching option. You may reserve your place on the waiting list with a $100 deposit that will be applied to your first month of coaching. Clients will be accommodated on a first-come, first-served basis.

If Jennifer cannot begin coaching with you within 90 days from the date that you make your deposit, you may choose to stay on the waiting list or have your deposit refunded.

Disclaimer of Liability

I hereby employ Jennifer Koretsky as “Coach” for the purpose of support surrounding my self-awareness, and personal and professional goals and challenges. I understand that she is not a psychotherapist, psychiatrist, medical doctor, or clinician and that she cannot diagnose any disorders/illnesses, nor provide medical attention/advice. I understand that coach/client communications are not legally privileged and court testimony may be required if a subpoena is issued. I waive and release Jennifer Koretsky and The ADD Management Group, LLC from any claims that arise or result from these services.  I have read this document carefully, and I understand and agree to all that has been stated.
